The monitor needs to be collected from Priory Medical Group - Heworth Green Surgery, 45 Heworth Green, Heworth, York YO31 7SX (open 8am-8pm) and delivered to her home, which is approx 1.2 miles away in Layerthorpe. Please note: The task is for Ms L who is COVID-positive but can be carried out at a physical distance as a doorstep delivery. No payment is needed.
